1. Child Immunization Checklist

Make an appointment with a healthcare provider, federally-qualified health center, rural health clinic or local health department. If your child is eligible for free vaccines through the Vaccines for Children (VFC) program, make sure to go to a VFC provider.
2. When you maintain a copy of your child’s immunization record:
Keep the record in a safe place where you can easily locate it.
Bring it to each of your child’s doctor visits.
Ask the doctor or nurse to jot down the vaccine given, date, and dosage on your child’s immunization record.
Write down the name of the doctor’s office or clinic where your child got the vaccine so you know where to get official records when you need them.
Ask if your healthcare provider participates in your state’s Immunization Information System (IIS). The IIS is a confidential digital record that keeps track of people’s immunization records. The IIS could help when you need an official copy of your child’s immunization record.
