1. Child Immunization Checklist

  • If you have questions about where to go to get vaccinations or about ways to pay for them, use these resources to find your answers; Vaccinations Locator and Help is Here

  • Make an appointment with a healthcare provider, federally-qualified health center, rural health clinic or local health department. If your child is eligible for free vaccines through the Vaccines for Children (VFC) program, make sure to go to a VFC provider.

2. When you maintain a copy of your child’s immunization record:

  • Keep the record in a safe place where you can easily locate it.

  • Bring it to each of your child’s doctor visits.

  • Ask the doctor or nurse to jot down the vaccine given, date, and dosage on your child’s immunization record.

  • Write down the name of the doctor’s office or clinic where your child got the vaccine so you know where to get official records when you need them.

  • Ask if your healthcare provider participates in your state’s Immunization Information System (IIS). The IIS is a confidential digital record that keeps track of people’s immunization records. The IIS could help when you need an official copy of your child’s immunization record.
